Step 1: You need Google Voice. It's still in the developing stages, so you need either an invite from a friend or from Google. You can sign up for one here. Even if you aren't interested in calling from your computer, you should still sign up for Google Voice. It's one of the best services Google has available; plus it's free. If you want to, you can set it up to forward calls to your mobile phone as well.
Step 2: Download and sign up for Gizmo5.
Step 3: Sign in to your Gizmo5 account. You should be in the account overview page. Copy your Sip number. Gizmo5 also has a Google Voice option, but don't enable it. It'll limit your phone calls to 3 minutes or you have to pay. Nobody wants that...

Step 4: In Google Voice, go to Settings. Then in the Phone tab, click "Add another phone". Match your settings to the picture below and paste the Sip number from Gizmo5 to the appropriate textbox. Save and make sure the "Forward To:" option is checked for this phone number.

Step 5 (optional): If you want, you can make your life even easier by installing the Google Voice Dashboard Widget(GV Connect) for Mac. It's really cool because you don't have to be on the Google Voice website to make a call. If you use Google contacts or Mac Address Book, GV Connect will sync with those so you have all your contacts available. Be sure to set your phone to ring to your Gizmo5 number.
TaDa! Now if someone calls your Google Voice number, it'll ring to your computer instead. And you can make calls out from your computer if you're too lazy to whip out your phone or if you want to save minutes. Simply, the Skype calling service, but free. Love it.
